Magnesium is a mineral your body needs for hundreds of everyday jobs, most of which happen without you noticing. Magnesium is a cofactor in more than 300 enzyme systems that regulate diverse biochemical reactions in the body, including protein synthesis, muscle and nerve function, blood glucose control, and blood pressure regulation, and it is required for energy production, oxidative phosphorylation, and glycolysis.
Most of it isn't floating around in your blood. An adult body contains approximately 25 grams of magnesium, with 50% to 60% present in the bones and most of the rest in soft tissues, while less than 1% sits in blood serum. That's why a simple blood test doesn't always tell the full story of your magnesium status.
In a typical Singaporean or Malaysian week — white rice at the kopitiam, char kway teow on a Friday night, ban mian for a quick lunch — magnesium-rich wholegrains, legumes, and leafy vegetables can easily get crowded out by refined carbs. It's not dramatic, just a quiet gap that builds up over time.
Magnesium helps muscles contract and then let go again, and it helps nerves fire signals properly. It plays a role in the active transport of calcium and potassium ions across cell membranes, a process that is important to nerve impulse conduction, muscle contraction, and normal heart rhythm. This is part of why it's often mentioned alongside muscle cramps and twitches after a hard workout, a badminton session, or a long day on your feet at the hawker stall.
Magnesium has a calming role in the nervous system. As an antagonist of calcium, magnesium binds to GABA receptors and activates GABA to reduce excitability of the nervous system, and it has been implicated in the regulation of sleep. Some studies looking at magnesium intake and sleep quality have found modest, encouraging links, though researchers are honest that many studies of the potential benefits of magnesium are quite small, and some have inconsistent results. Think of it as one piece of a good wind-down routine, not a standalone fix for sleepless nights.
Bones are where most of your body's magnesium actually lives, so it makes sense the mineral matters for skeletal health. Inadequate dietary intakes and/or low serum concentrations of magnesium have been associated with increased risk of cardiovascular disease, osteoporosis, and metabolic disorders. Paired with calcium and vitamin D, magnesium supports the structural framework that keeps bones dense as you age — relevant for anyone whose diet leans heavily on white rice and noodles with little dairy or leafy veg.
Magnesium is also tied to how your body handles sugar and keeps blood pressure in check. Low magnesium status has been associated with increased risk of metabolic disorders including metabolic syndrome, hypertension, and type 2 diabetes, and magnesium supplementation may improve insulin sensitivity in individuals at risk for type 2 diabetes. This is worth noting locally, given how common diabetes screening and blood pressure checks are through HPB health programmes in Singapore and similar initiatives in Malaysia.
Magnesium is absorbed mainly in the small intestine, and your kidneys do the heavy lifting to keep levels steady. Magnesium homeostasis is largely controlled by the kidneys, which typically excrete about 120 milligrams of magnesium into the urine each day, and urinary excretion is reduced when magnesium status is low. In other words, a healthy body is constantly fine-tuning how much it keeps versus flushes out.
A few things can get in the way of absorption. Long-term use of certain heartburn medications is one of them: prescription proton pump inhibitor drugs can cause hypomagnesemia (low magnesium) when taken for prolonged periods, typically more than a year. Research suggests this happens because intestinal malabsorption of magnesium is the main cause of PPI-related hypomagnesemia, as PPIs change conditions in the gut that affect how magnesium is absorbed. If you've been on long-term reflux medication, this is worth a mention at your next check-up.
Interestingly, magnesium is naturally present in many foods, added to other food products, available as a dietary supplement, and present in some medicines such as antacids and laxatives — so some people are already getting incidental magnesium from common over-the-counter remedies without realising it.
Magnesium from food is generally very well tolerated. High-dose supplements can lead to diarrhea, nausea, and cramping in some people, while extra magnesium from food is safe because the kidneys will eliminate excess amounts in urine. This is a useful distinction — the food source and the supplement source don't carry the same risk profile.
Certain groups need to be more careful. People with chronic kidney disease should not take magnesium supplements unless prescribed by their doctor, since impaired kidneys can't clear excess magnesium as efficiently. Older adults, people with digestive conditions that cause chronic diarrhoea, and anyone on long-term acid-reducing medication or certain diuretics should also flag magnesium supplement use with a doctor or pharmacist rather than self-prescribing. If you're already taking a laxative or antacid, check the label — you may be getting more magnesium than you realise.
The good news: legumes, nuts, seeds, whole grains, and green leafy vegetables such as spinach are among the foods that provide recommended amounts of magnesium, and most of these fit naturally into SG/MY meals with a few small swaps.
